Pathways HOME

Pathways HOME provides safe and secure housing and wrap-around services for homeless young adults between the ages of 18 to 24 years old.

The program is primarily funded via several unique grants through the Federal Department of Housing and Urban Development (HUD).

New referrals are accepted via the Coordinated Entry team maintained by the Louisville Continuum of Care at 502.637.2080.  Housing assistance is limited to those who currently meet the Federal HUD homeless definition.

All new housing referrals are required to come through Louisville’s Coordinated Entry System at 502.637.2080.

If you are currently homeless and between 18 to 24 years old, please call 502.637.2080 to start the assessment and referral process.

 

Pathways HOME also administers the Youth Action Board, comprised of current and formally homeless young adults. If you are interested in becoming a member of the Youth Action Board, please call 502.596.1320.
