Kentucky Impact Plus Case Management
What is Impact Plus?
IMPACT PLUS offers hope to children who have complex mental health and behavioral needs. IMPACT PLUS teams family, community supports and providers to stabilize the child’s behaviors so they can remain in their homes.

Services
Case Management - Parents or caregivers, as well as other service providers, form a team to develop a service plan. This team coordinates services and helps access appropriate community resources.
Behavioral Health Evaluation – Face to face evaluation and diagnosis.
Individual Therapy - Intensive one on one service provided by a licensed behavioral health professional to address behavior and mental health needs of the child.
Collateral Services - Assist caregivers in meeting the child’s needs through face to face services.
Therapeutic Child Support - One on one services with recipient and/or caregivers providing in-home support, behavior management, social skills development and assistance with like transitions.
After School or Summer Program - Individual and group activities with therapy and individual TX plans.
Partial Hospitalization - Less than 24-hour care, 5 to 7 days per week with management by psychiatrist; therapy and medications.

Who is eligible?
A Medicaid recipient under the age of 21 may be eligible to receive IMPACT PLUS if they:
* Are in the custody or under the supervision of the state; OR are at risk of being in the custody of the state; OR are currently in a hospital or psychiatric residential treatment facility and would be at risk of institutionalization if discharged.
* Have a DSM-IV multi axel diagnosis
* Have emotional/behavioral problems that have persisted in the home, school or community in the last six months.
* Have a current medical card.
